Support Coordination

As part of your NDIS plan, you may receive funding for a Support Coordinator.

 What is support coordination?

Once you have a NDIS plan you can choose who your service providers are. This can be stressful and hard to coordinate yourself. At Care to Change we have professional Support Coordinators who are able to assist you with connecting with service providers and allied health professionals that will meet your needs.


How does Support Coordination become part of my NDIS Plan?

In addition to the support provided by family, friends, and other community and government services, your funding will be determined by what is ‘reasonable and necessary to work on and reach your goals.

Budgets for capacity building will include support coordination where it is reasonable and necessary. To use your plan, you must pay a fixed amount for a support coordinator.

Your plan will describe the level of support coordination that is funded, and you can only purchase this level of support coordination.

  What can my Support Coordinator help me with?

Support Coordinators can connect you with different services that you need in order to work towards or meet your NDIS Goals. They also assist you with:

  • Identify services that meet your needs
  • Coordinate service providers and support
  • Liaise with agencies and other providers
  • Find suitable accommodation and respite options
  • Link you to work within your community
  • Link you with activities within your community
  • Implement and monitor your NDIS plan
  • Understanding your NDIS Plan
  • Establishing the best way to use your funds to meet your needs
  • Organising appointments and referrals for Allied Health Professionals and obtaining and reviewing reports
  • Prepare evidence for Plan Reviews

Levels of Support Coordination

There are three levels of support coordination that may be included in your plan:


Support connection

It will enable you to get the most from your plan an pursue your goals, by connecting you with informal, community and funded supports.

Support coordination

This helps you to understand and make the most of your plan. A support coordinator will work with you to make sure that a variety of supports are used to help you maintain relationships, manage services, and be independent and integrated into your community.

Specialist support coordination

The coordination of support is on a higher level. These services are for people whose situations are complex and who require specialist support.
Your Support Coordinator will assist you in managing challenges in your support environment and ensuring consistent service delivery.
